With the confirmation of the pregnancy of the emperor's favorite consort, Lady Gyokuyo, Maomao returns to the Jade Palace as his poison tester. While vigilantly preventing further attempts on the consort and the imperial heir, several other issues begin to arise: the arrival of a new consort, Lady Loulan, daughter of a powerful minister from the former emperor's era; an unsolved case that put Jinshi's life at risk; and the unknown whereabouts of the suspect, Sui Rei. However, amidst this tense atmosphere, foreign merchants and an emissary arrive at the palace with absurd demands.

However, Kusuriya no Hitorigoto ( Diaries of an Apothecary ) began its publication in October 2011 in Shousetsuka ni Narou . Eventually the novel had two manga adaptations, the first by Nekokurage published in Big Gangan and the second in Sunday Gene-X.
