It turns out that the villain we saw in the anime isn't Geto, but rather a curse that stole his body.

In the second season of Jujutsu Kaisen, it was revealed that Geto was a student at the jujutsu sorcerer academy and a friend of Gojo. However, he questioned his purpose after the death of Riko Amanai. Geto decided to exterminate all ordinary humans to create a world of only sorcerers. Since then, he lived as a villain and enemy of the sorcerer academy until he confronted Yuta.
After facing Yuta, Geto was severely injured and unable to move, so Gojo seized the opportunity to kill his enemy. The main story takes place a few years after the events of Jujutsu Kaisen 0, and Geto is still alive, which might lead one to believe he somehow survived. However, Geto actually died, and his body now serves as a vessel for a curse called Kenjaku.
Kenjaku is a curse that has been alive for centuries, and it usually takes the form of a brain to control a sorcerer's body as if it were its own. When Geto died, Kenjaku took over his corpse to have a new body. Thus, it can use Geto's abilities and body at will, but no one knew about its existence until the Shibuya battle.
Synopsis:
The story follows Yuji Itadori, a high school student who becomes involved with a secret society of sorcerers called Jujutsu Kaisen, who fight against curses, evil beings that haunt the world of the living. To protect his friends and innocent people, Yuji decides to join the Jujutsu sorcerers as a trainee under the tutelage of Satoru Gojo, the strongest sorcerer of them all.
